In all fairness, the tether tests went fine. NASA TV is showing those, and the engine appears to be running well. Pay attention to the nozzle in the video and you'll notice it looks to be hard-over in the direction of the arc. Gimbal control may have failed; there was plenty of thrust, although that may have been cut when the machine started tipping past critical.

So, nine minutes before the fire trucks start spouting foam? Are they taking lessons from the Cleveland Fire Department?

Well...at least they had firefighting personnel and equipment on hand...

The rocket burned through its supports and fell over on the launch pad (576A3, an old Atlas pad), and its SDI payload broke off and rolled away. Because the old launch pad had no working sprinkler or deluge system, the rocket burned in a slow, smoldering fire for the next day. The base commander said that it was the most benign launch failure he had ever seen.
